Пассивный залог используется тогда, когда нам НЕ ВАЖНО, КТО совершает действие. ВАЖЕН САМ ФАКТ совершения действия.
Mulder was spotted in the secret base. - Малдера заметили на секретной базе.
Неважно, кто его заметил. Важно, что его заметили.

В схеме части A, сначала идет вспомогательный глагол to be, а потом смысловой глагол Ving, тоже to be

Когда надо указать, кем совершается действие
Хотя тот, кто совершает действие, в пассивном залоге не важен, мы всё-таки можем указать его при помощи by.
Mulder is criticized by colleagues. - Малдера критикуют коллеги.
Можно сказать, что by прицепляет творительный падеж (кем?), когда действие совершается одушевлённым предметом.

Когда речь идёт об инструменте, который мы используем для совершения действия, используем with.
The distress signal will be given with a flare gun. - Сигнал бедствия будет подан при помощи ракетницы.
Можно сказать, что with прицепляет творительный падеж (чем?), когда действие совершается неодушевлённым предметом, который используется как инструмент.
